As a member of the Council of Community Services you will have access to our Delta Dental Benefit Programs. For a minimal fee CCSNYS can assist you in offering and managing a dental insurance plan(s) for your nonprofit. The Delta HMO.  The Delta HMO is a prepaid managed care plan. Members must first elect a participating primary care dentist to receive services from. Members are able to change primary care dentists but must do so prior to receiving services by another PCD. The members will receive a schedule of benefits which list the fees for specific services; the fee is the amount in which the member has to pay for the service(s) rendered. The Delta HMO plan has no deductible and no annual maximum. This plan works in network only; if a member receives services from a nonparticipating dentist including any type of specialist without prior approval the claim will not be paid. Exceptions can only be made by obtaining prior written authorization from Delta Dental. Click here for a comprehensive list of benefits.

Delta Dental Traditional Plan. The Delta Traditional plan allows for the freedom and flexibility of obtaining your services from an in or out of network dentist. Employees that are eligible to enroll and do so will become effective the 1st of the month following 60 days from date of hire. If going to a participating provider the member will only pay the scheduled percentage amount. When services are provided by a nonparticipating dentist Delta will reimburse the member up to the scheduled allowance it is the member’s responsibility to pay the difference between the scheduled allowance and the dentist’s fee. All covered dental procedures except preventive & diagnostic services are subject to an annual deductible of $50 for each individual, not to exceed $150 per family. The annual maximum allowed per person is $1000; any unused amount at the end of the year will not rollover to the following year.  

Once your agency is an active Council member all eligible employees may enroll in the plan effective the first of any month. If an employee chooses to waive the coverage and decides at a later date that they are in need of dental coverage the member would have to wait until the next open enrollment period.
